SportsLab ("we," "us," or "our") is a sports performance training and rehabilitation center committed to protecting your privacy. This Privacy Policy describes how we collect, use, disclose, and safeguard your personal information when you visit our website, use our services, or communicate with us, including via SMS text messaging. Please read this policy carefully.
1. Information We Collect
We may collect the following types of information:
Personal Information:
- Name, date of birth, and contact information (address, phone number, email)
- Insurance information (including Aetna, Blue Cross Blue Shield, First Choice, and Cigna) and billing details
- Medical and health history relevant to your care and treatment
- Mobile phone number, if you opt in to SMS communications

2. How We Use Your Information
We use the information we collect to:
- Provide, schedule, and coordinate your physical therapy, rehabilitation, massage therapy, sports performance, and related healthcare services
- Send appointment reminders, follow-up care instructions, and billing communications via SMS, email, or phone
- Process insurance claims and payments
- Coordinate care among SportsLab practitioners to ensure comprehensive treatment
- Comply with applicable federal and state healthcare laws, including HIPAA
- Improve our website and patient experience
- Respond to your inquiries and requests
3. SMS/Text Messaging Communications
A2P Messaging Compliance: SportsLab complies with all applicable A2P (Application-to-Person) messaging regulations. This includes obtaining explicit consent from patients before sending any SMS messages, informing you of message frequency, applicable costs, and how to opt out.
If you provide your mobile phone number and consent to receive SMS messages from SportsLab, we will use your number to send appointment reminders, care updates, billing notices, and other relevant communications.
Message and data rates may apply. Message frequency varies based on your appointment schedule and care plan.
Consent to receive SMS messages is not a condition of receiving care from SportsLab.
Data Retention: We retain your mobile phone number and SMS opt-in information for as long as you are an active patient or remain subscribed to our SMS communications. You may request deletion of this information at any time by contacting us directly.
You may opt out at any time by replying STOP to any message. You will receive one confirmation message and no further texts will be sent. To restart text communication, text START to any message.
SportsLab will not sell, rent, or share your mobile phone number or SMS opt-in information with any third parties or affiliates for marketing or promotional purposes.
Carriers are not liable for any delayed or undelivered messages. SportsLab is not responsible for delays or failures caused by your mobile carrier or any third-party network.
4. How We Share Your Information
We do not sell or rent your personal information. We may share your information in the following limited circumstances:
- With SportsLab practitioners and care team members involved in your treatment
- With third-party service providers who assist in operating our practice (e.g., scheduling, billing software), subject to confidentiality obligations
- With insurance carriers and payers as necessary to process claims
- With shared administrative staff who operate within our facility and have completed HIPAA training
- As required by law, including in response to lawful requests from government authorities
- To protect the rights, safety, or property of SportsLab, our patients, or others
Any sharing of protected health information (PHI) is performed in accordance with HIPAA and applicable Oregon state law.
5. SportsLab and Athletic Republic
SportsLab and Athletic Republic are operated under common legal ownership and co-habit the same facility, sharing certain administrative staff. All administrative staff with access to SportsLab patient data have completed HIPAA training and are subject to our privacy practices.
Athletic Republic coaching staff who have no clinical or administrative role within SportsLab do not have access to SportsLab patient records, health information, or contact data.
Your personal health information, treatment records, and contact details collected by SportsLab will not be used for Athletic Republic marketing purposes, nor will your clinical or health information be accessed by Athletic Republic staff, without your express written consent.
If you wish to share your information with Athletic Republic for the purpose of a coordinated training referral or program, written consent is required. This may be provided via a consent checkbox on your intake form or a separate written authorization submitted to any SportsLab staff member.
6. HIPAA and Protected Health Information
As a healthcare provider, SportsLab is subject to the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information (PHI) is handled in accordance with our Notice of Privacy Practices, available upon request at our clinic.
Your key rights under HIPAA include:
- The right to access and request a copy of your health records
- The right to request amendment of inaccurate records
- The right to request restrictions on how your PHI is used or disclosed
- The right to request confidential communications at a specific address or phone number of your choosing
- The right to an accounting of certain disclosures of your PHI over the past six years
- The right to receive notification if your unsecured PHI is breached
- The right to file a complaint without fear of retaliation
SportsLab will not retaliate against you in any way for filing a privacy complaint.
7. Breach Notification
In the event of a breach of your unsecured Protected Health Information, SportsLab is required to notify you as soon as possible, and no later than 45 days following discovery of the breach. Notification will be made by first-class mail or email (if you have indicated a preference for email communication).
The breach notification will include:
- A description of the breach, including the date of the breach and the date of discovery
- A description of the type of unsecured PHI involved
- Steps you should take to protect yourself from potential harm
- Actions SportsLab has taken or will take to investigate and mitigate the breach
- Contact information for follow-up questions

SportsLab will not retaliate against any patient who files a privacy complaint.
9. Data Security
We implement reasonable administrative, technical, and physical safeguards to protect your personal information from unauthorized access, use, or disclosure. However, no method of electronic transmission or storage is completely secure, and we cannot guarantee absolute security.
10. Cookies and Website Tracking
Our website may use cookies and similar technologies to enhance your browsing experience and analyze website traffic. You may disable cookies through your browser settings; however, some features of our website may not function properly without them.
11. Children's Privacy
Our services are not directed to children under the age of 13. We do not knowingly collect personal information from children under 13. If you believe we have inadvertently collected such information, please contact us and we will promptly delete it.
12. Changes to This Privacy Policy
We may update this Privacy Policy from time to time. Changes will be posted on this page with an updated effective date. If we make material changes, we will provide updated notices to current patients. We are required to maintain each version of our Privacy Policy for a minimum of six years.
